Hi Anna, I sometimes prefer to hold the shoulder rest with my arm folded so my head can rest on my upper arm and sometimes I prefer my arm flat. I don't have neck issues so I tend to aim for comfort for myself. I look for alignment as well as comfort when my clients are in this position (and others). I wouldn't hesitate to add a towel or incline the head rest if it suited the client's neck issues and prevented pain. Glad you liked the class.
